Breast Ultrasound Scan – Introduction
Our specialists perform both targeted breast ultrasound for specific lumps or areas of concern and whole‑breast ultrasound when a comprehensive evaluation is required. When indicated, we also assess the axilla (underarm lymph nodes) to provide a complete diagnostic picture. Each scan is carried out using high‑resolution probes and orthogonal imaging planes, allowing us to identify and characterise breast tissue and any pathology with precision.
Findings are described using recognised benign, indeterminate and malignant ultrasound features — such as lesion shape, margins, echogenicity and sound transmission — ensuring your report is clinically robust and easy for your healthcare provider to interpret. As your document states, “Measurements of identified lesions are to be recorded in 3 dimensions… with the measurement in millimetres.”

Treatment Overview
Most breast lumps are benign, meaning they are not cancerous, and ultrasound is an excellent way to investigate them gently and accurately.
A breast ultrasound uses high‑frequency sound waves to create detailed images of the tissues beneath the skin. The scanner sends sound waves into the breast, which then reflect back from different structures.
By detecting these subtle variations, the ultrasound system builds a precise picture of the breast’s internal architecture, helping clinicians assess any lump or area of concern with clarity and confidence.

Clinical Indication:
Ultrasound should be used as part of the assessment of any clinically suspicious or discrete/indeterminate findings within a clinic. 28. NHS Breast Screening Programme Clinical guidance for breast cancer screening assessment describes the criteria for entering the Rapid diagnostic assessment pathway and the referral process within.
US of the axilla if a suspicious breast lesion is identified or any symptomatic lump.
US may be used as an alternative modality whilst breastfeeding, and
Mammography should be delayed for at least 3 months after cessation of lactation.
Imaging Technique
Patients are asked to put the gown on with the opening at the back unfastened, to enable the sleeve of the gown for the relevant side to be slipped off easily, whilst covering the other breast for dignity/warmth.
Position the patient appropriately on the couch, utilising a positioning wedge as required.
Targeted US is performed of the relevant quadrant of the breast as indicated by the referral and during response to treatment scans at the site of the known lesion.
Whole breast US is performed for annual surveillance and when there is a suspected malignancy at diagnosis. If a suspicious pathology is identified within the breast the axilla should also be scanned.
Scanning should be performed in orthogonal planes.
Initially select the 2-14MHz probe, change the frequency, power, depth of the probe or change to the 2-9MHz probe in the case of a large-breasted or dense breast tissue patient.
Anatomical Evaluation
Describe the features of any pathology utilising the malignant/benign features descriptors below. Measurements of identified lesions are to be recorded in 3 dimensions: transverse x depth x longitudinal planes, with the measurement in millimetres.

Reporting Format
• Reported and authorised on our system with grading.
• Describe the breast tissue, involuted glandular, etc.
• Describe any identified pathology and its location within the breast using the clock face vector and distance from nipple.
• If axillary scanning is performed, describe the:
- Absence of lymphadenopathy
- Abnormal-appearing nodes and the volume. Greater than 4 abnormal-appearing nodes
Grade the findings
• U1 Normal
• U2 Benign: no suspicious findings.
